Supported Community Living Program

MI Rehab Solutions offers a Supported Community Living Program (SCLP) to meet adult (ages 18+) individual client’s needs in establishing and maintaining their optimal level of functional independence in the least restrictive environment and providing supports/services, ensuring safety while striving to unleash the potential of every individual served.

Living Environment:

  • Wheelchair accessible/barrier free environment, as appropriate, based on client needs
  • Access to public transportation, as appropriate
  • Access to fitness center
  • Access to 24/7 emergency support system



Program Supports/Services:

Rehabilitation Therapy Services:  
  • ​Comprehensive Therapy Initial Evaluations (Physical Therapy, Occupational Therapy, Speech/Language Pathology, Social Work and Therapeutic Recreation Services)
  • Monthly Monitor Sessions for Active Rehabilitation Therapy Services to assess rehabilitation goal status, Plan of Care (POC) updates, and to provide ongoing Rehab Specialist supervision and support as appropriate.
  • Ongoing therapy services as determined, recommended, prescribed by physician, to be billed at individual therapy rates; separate from program fee/per diem
Rehab Specialist Services: 5 to 7 days/week, provided under supervising therapist/clinician plan of care.  Supports/services may include, and are not limited to, the following:
  • Schedule Oversight/Calendar Management
  • Medication Monitoring
  • Home Maintenance / Environment Monitor: chore list completion, guest status, smoking/non-smoking, general safety considerations/access 
  • ADLs/iADLs follow-through
  • Meal Planning/Prep Support
  • HEP completion/follow-through

Money Management: (Variable dependent on intake assessment and client needs).  Supports provided as recommended based on evaluation findings, including:
  • Budget oversight/tracking 
  • Distribution of allocated funds from guardian/conservator 
Medication Management: (Variable dependent on intake assessment and client needs)
  • Set-up of medication organization
  • Support/monitoring that medications are taken as prescribed
  • Support/facilitation of refill communication
  • Drug/Alcohol Screens; frequency to be determined as needed

Internal Case Management Services: 
  • Intake assessment
  • Facilitation of therapy evaluations and progress reports
  • Communication with external team: physicians, case managers, insurer, other external providers as appropriate
  • Coordination of all therapy services and scheduling
  • Monitor and review of program requirements and carry-over/implementation by client
  • Facilitation of team meetings
  • Monthly ICM report

Program Timeline:

INITIAL EVALUATION PHASE​​ (12 weeks)

New clients requiring comprehensive evaluations and determination of appropriate supports/services to live at optimal level of functional independence in least restrictive environment.
*Qualifying clients require less than 24 hours/day of attendant care.

SHORT-TERM/ TRANSITION PHASE (< than 6 months)

Clients requiring short term/transition support, and/or with anticipated services / needs known. This may include clients discharging from acute rehabilitation inpatient program, awaiting appropriate access, safety, or support services to be in place prior to return to his/her previous living location/arrangement.
*Qualifying clients require less than 24 hours/day of attendant care.

LONG-TERM/ TRANSITION PHASE (> than 6 months)

​ Clients requiring long term/transition support or those who have completed Initial Assessment Phase and continue to require a supported community living program.
*Qualifying clients require less than 24 hours/day of attendant care.
