Home & Community-Based Rehabilitation Services

Our team works alongside clients of all age (pediatric 0-11, adolescent 12-17, and adult 18+), families, and other support systems to identify and address factors impacting recovery.   We facilitate individualized solutions to help each client reintegrate to home, work, school, and other relevant environments.

Our comprehensive services address clients’ needs at any point along the continuum of care, from post-acute evaluation until the time that support and services are no longer required, ensuring transition to community resources as appropriate.

PHYSICAL THERAPY

​Diagnose and treat individuals of all ages to address limitations in the ability to move and perform functional activities.  Using treatment techniques, a plan will be established to promote the ability to move, reduce pain, and restore function.

OCCUPATIONAL THERAPY

Evaluate and treat limitations that make it difficult for individuals of all ages to engage in everyday activities.  Through the therapeutic use of everyday activities, as well as other skilled interventions, facilitate function in all environments.

  • Sensory Diet
  • Orthotic Fabrication / Fitting 

SOCIAL WORK/ COUNSELING

Provide supportive counseling, facilitates coping and adjustment to changes status post injury, as well as provide resource to individuals who need help connecting to government or community supports.

  • Client/Family Support/Counseling
  • Facilitation of SSI, SSDI, Government/Health Insurance Benefits
  • Overview of housing resources and eligibility for subsidized assistance
  • Transportation Overview/Resources

THERAPEUTIC RECREATION

Improve or maintain physical, cognitive, social, emotional and spiritual functioning in order to facilitate full participation and quality of life using treatment services and recreation activities including arts and crafts, animals, sports, games, dance and movement, drama, music, and community outings. Recreational therapists treat and help maintain the physical, mental, and emotional well-being of their clients by seeking to reduce depression, stress, and anxiety; recover basic motor functioning and reasoning abilities; build confidence; and socialize effectively.

SPEECH-LANGUAGE PATHOLOGY

Assess, diagnose, and treat speech, language, social communication, cognitive-communication, and swallowing disorders in children and adults.
